  1. The LP Accreditation application form+

    Make sure you fill in this form before you complete your submission.

  2. Evidence folder - click on this to see the organising structure of folders where you should upload your portfolio evidence.

  3. Link to Reference Library - links to pages which explain the Lead Practitioner process.

  4. Link to submit your application. Only click on this when you are sure you have uploaded all your portfolio and completed the application form fully. See Submit your portfolio
